By sending the completed form, the data subject provides his/her personal data and consent to the processing thereof for the stated purpose to the operator, which is LUSSUOSA, a.s., with registered office at sídlom Štúrová 12, 811 02 Bratislava, company registration number: 48 287 130, registered in the Commercial Register of District Court Bratislava I, section: Sro, insert no. 106108/B (hereinafter also referred to as the “Operator”).
